We, the student & guarantor, if applicable, on behalf of ourselves, members of our family, our heirs, executors, administrators and assigns, hereby forever release, discharge and hold harmless, Toronto Striking Academy Inc., representatives and agents for any injury, loss, or damage to my person or property howsoever caused, arising out of or in connection with my taking part in martial arts or fitness classes and activities and notwithstanding that the same may have been contributed to or occasioned by the negligence of Toronto Striking Academy Inc. representatives or agents. Please Note: participants must supply their own protective equipment. We do not offer any refunds or transfers.
Furthermore, I, the participant and/or parent/guardian, grant permission to Toronto Striking Academy and Team Havoc Corp. to photography and/or record me or my child/ward’s image and/or voice on still or motion picture, film, and/or audio tape, and to use this material to promote Toronto Striking Academy and Team Havoc Corp through the media of newsletters, websites, television, film, radio, print and/or display form. I understand that the audio/visual material and copyright will remain the sole property of Toronto Striking Academy and Team Havoc Corporation and I waive any claim to remuneration for use of audio/visual materials used for these purposes. We do not sell or distribute your personal information to any other third party not listed herein.
